Fill Out an Application

If you are ready to claim a doodle puppy, please fill out the application below. I am very prompt at answering texts and emails.  If you do not hear from me within 5 hours of submitting your application, please call 320-250-2464 and I will do a verbal app with you! To hold the puppy, you'll also need to make a deposit. I will do that over the phone. For your initial deposit, I only take a credit card. You may make your final payment in cash, if you wish, or the remainder can go on a credit or debit card. WHAT IS THE BEST TIME TO CALL YOU???  PLEASE WRITE IT WITHIN THE OCCUPATION LINE!!  THANKS!
